New Patients


We always welcome new patients and we would be pleased to have the opportunity to provide excellent dental care for you. Your family, friends, neighbours, and co-workers are also welcome.


If you are visiting our office for the first time, we ask that you arrive about 15 minutes early to complete a new patient questionnaire and register with our friendly and knowledgeable front staff. We also require your dental insurance information [policy/contract number and ID/certificate number] at least one day before your appointment in order for us to confirm your dental coverage information. It is important that you make certain that your dental insurance policy and personal information are always current.



Transfer of Diagnostic Records


If you are a new patient coming to our office for the first time and you have had recent X-rays taken with your previous dentist, we ask that you provide us with the name of your previous dentist and the dental office so we can request your diagnostic records. We require that you sign a release form, giving your consent to transfer your diagnostic records to our office.



Appointment Policy


If you are unable to keep a scheduled appointment, we ask that you provide us with at least 24 hours notice so that your previously reserved time may be given to another patient. Otherwise a charge will be applied for missed appointments and short notice cancellations.



Pre-Booked Appointments


For those previously scheduled appointments booked more than three months away, our dedicated front staff will give you a reminder call two weeks before the scheduled appointment to pre-confirm. Our front staff will give you a maximum of three reminder calls and if you failed to confirm to confirm your appointment two days prior to the schedule time we will cancel your appointment .
